From the moment someone moves to our home, we take the time to get to know them—their life story, routines, values, interests, and goals. This information helps our team create a personalised care plan called ‘Key to Me’ that reflects who they are and what matters most to them.

Each resident’s personalised ‘Key to Me’ care plan is regularly reviewed and updated to reflect their evolving health needs, personal preferences, and individual care goals.

 

Why Is Person-Centred Care Important?

Multiple studies have shown that person-centred care improves quality of life, promotes independence, and supports overall wellbeing in older adults. It allows residents to retain a sense of control, purpose, and dignity—even as their health needs change.
